Student Organization Officer Change Form

Organizations may use this form any time they have changes in their officers during the academic year.  This form should only be utilized once the full organization registration has been completed for the academic year.  If you are unsure if your organization has completed a registration for the 2015-2016 academic year, contact Michelle Welter at mking@siue.edu.

Please note: If some officers will remain the same, you do not need to resubmit their information on this form.  Only submit officers that are changing.  If an individual held one position and is now going to hold a new position, please submit their information for their new position.
